The following resources may be used to assist with collecting the appropriate documentation for proof of residence.

Enrollment and Registration Information
Parents and guardians looking to register their child in any of our k-12 schools will first need to complete the appropriate online registration forms. This online enrollment process will allow us to collect all necessary personal and contact information about you and your student.

Enrollment Process and Requirements
To begin the enrollment process click the link above and enter all of the necessary items in the form presented to you. Completing the first step will deliver an email to the account you provide with a link that will allow you to continue registration or resume registration if you require multiple sessions to complete the form. If you do not have an email address, please visit one of the registration kiosks available in each of our schools and Central Office or call your school to schedule an appointment with the registrar.

Once you begin the online registration, follow the on-screen instructions to complete all required components of the form. Afterward, contact your school to schedule an appointment to complete registration.
The following types of questions will be asked in the application:

Household information (address and phone numbers)

Parent information (work and cell phone numbers, email addresses)

Student information (demographic and health/medication information)
-
Emergency Contact (addresses and phone numbers)
In addition, applicants new to Baldwin County Schools or returning after having withdrawn will need to upload the following documents to the application:

Birth Certificate (certified copy)

Social Security Card
-
Immunization Record (Georgia form 3231, current/up-to-date)

Ear, Eye, and Dental screening (Georgia form 3300)

Current Proof of Residence (lease or mortgage statement/property tax records in parent's/legal guardian's name)

Current Utility Bill (power, water, gas, cable in parent's/legal guardian's name)

Parent/ Guardian Picture ID
If the utility bill/lease is in someone's name other than the parent/legal guardian, the following documentation must be presented in addition to the above documents:

Notarized affidavit from the person whose name is on the POR/utility bill stating that the parent/legal guardian and child live at the address indicated on the POR/utility bill.This affidavit can be picked up at the school your child is zoned for. This letter must be notarized by a licensed notary public in order to be accepted.
Note: Required fields are marked with a red asterisk, and the district will receive the data exactly as it is entered. Please be careful of spelling, capitalization and punctuation. Dates should be entered as MM/DD/YYYY and phone numbers as xxx-xxx-xxxx.

Students that have relocated and are requesting a school transfer will need to present 2 new proofs of address to the transferring school.
